Headache from the neck? How to read the pattern

By Updated 5 September 2026

Your head hurts, and your neck feels tight too. They may be connected. But a tight neck does not tell us what type of headache you have. I would start with what feels familiar, what has changed, and whether this needs medical attention before self-help.

Can a headache come from the neck?
My approach: alternative treatment, not a diagnosis

I use lymphatic treatment as an alternative working model involving muscle, connective tissue and a before/after observation. Words such as pump and passage describe my approach. A symptom change does not prove that lymph flow caused the pain. Clinical manual lymphatic drainage for lymphedema is a different care pathway.

Short answer

Yes, pain referred from the neck can be felt in the head, but headache type cannot be confirmed by one pressure point or online test. A neck-related pattern is more plausible when neck movement is limited or reproduces the familiar headache. New, sudden or unusual headaches need a safety check before self-help.

LOCATION AND BEHAVIOUR

Common patterns to compare

The same symptom label can describe different patterns. These clues help you choose a sensible next step; they do not confirm a diagnosis.

Starts in the neck or back of head

The pain may spread toward one side of the head, temple or behind the eye.

Changed by neck movement

Restricted movement or reproduction of the familiar pain supports a neck contribution but does not prove it.

Migraine-like features

Nausea and light or sound sensitivity can overlap. Do not assume every headache with neck tension is cervicogenic.

A PRACTICAL PLAN

What to do today

  1. Rate the familiar headache and gently turn the head left and right—do not force the range.
  2. Use only a comfortable self-help step away from the front and side of the neck, then repeat the same movement.
  3. If the headache is recurrent, changing or difficult to classify, keep a symptom diary and discuss it with a healthcare professional.
Use one consistent measure

Choose one movement that safely reveals the signal. Rate it from 0–10, change one thing, then repeat the same movement. Also note the response later that day and the next morning.

PAINFREEU'S ROLE

Measure first. Change one thing. Measure again.

PainFreeU treats the response as a clue, not a diagnosis. Avoid hard pressure on the front or side of the neck. Stop if pressure causes dizziness, visual change, nausea, numbness or a stronger headache.

PFU Pocket is general education and self-help. It does not diagnose the cause, replace a clinician or guarantee pain reduction.

WHEN SELF-HELP IS NOT ENOUGH

Warning signs

Call emergency services for a sudden “worst ever” headache, headache after significant injury, or headache with weakness, confusion, fainting, seizure, fever and stiff neck, loss of vision or difficulty speaking. New headache during pregnancy or with cancer/immunosuppression needs prompt advice.

If you are uncertain, choose assessment over another online test. In an emergency, contact your local emergency service.

QUICK ANSWERS

Frequently asked questions

What does a cervicogenic headache feel like?

It often begins in the neck or back of the head and can be changed by neck movement, but symptoms overlap with other headaches.

Can I diagnose it by pressing my neck?

No. Reproducing pain may be informative, but it is not a valid standalone diagnosis.

Should I stretch a stiff neck during a headache?

Gentle comfortable movement may help some people. Stop if it increases head pain or causes neurological symptoms.
